Admissions Process

When you or a loved one needs rehabilitation services, you may not know how to select the appropriate facility. This page walks you through the process.

 

The Referral

Most of our patients come to our facility directly from another hospital. The referring hospital often has a case manager who invites two to three rehabilitation hospitals to assess a patient and offer a care plan.

Twin Creeks Hospital’s clinical liaisons visit with the patient, family, and the hospital staff to collect pertinent information. Twin Creeks Hospital’s medical team uses this information to determine the patient’s potential and desire to participate in an intensive course of rehabilitation therapy. We also understand the importance of families and caregivers to our patients’ overall success, so we include them in this process, as appropriate, to ensure everyone understands the goals and process for treatment.

At this point the hospital either accepts the admission of the patient or makes a recommendation for an alternate level of care. However, once accepted, the patient and patient’s family make the final choice about which rehabilitation hospital best meets their needs.

Initial Assessment

Twin Creeks Hospital uses a standard, objective tool to measure our patients at admission and at discharge. To date, our measure outcomes exceed industry standards.
